Apply for Admission to MTSU<\/a><\/td>\n Complete the online Dual Enrollment Application. Dual Enrollment students will apply as Undergraduate students<\/td>\n<\/tr>\n\n
Submit Documentation for Eligibility Verification for Entitlements Act (EVEA)<\/td>\n Upload EVEA document within your MTSU application or submit within your MTSU application portal after you\u2019ve completed the application<\/td>\n<\/tr>\n\n
Submit the Application Fee<\/td>\n There is a non-refundable $25 application fee (part of the Admissions application)<\/td>\n<\/tr>\n\n
Submit Official High School Transcript<\/td>\n High School and College officials<\/strong> can use Slate.org<\/a> to send transcripts or we accept any electronic transcript services. Official Transcripts can also be mailed to: MTSU Undergraduate Admissions SSAC 1201301 E. Main St.Murfreesboro, TN  37132

*Transcripts may not be submitted to MTSU via email.  To protect the personally identifiable information of prospective students, any transcript sent via email will be deleted and will not be processed.<\/td>\n<\/tr>\n\n
Submit Official ACT\/SAT\/Pre-ACT<\/td>\n Students who have not completed one of these tests must register for ACCUPLACER (MTSU Placement Test)<\/td>\n<\/tr>\n\n
Apply for Dual Enrollment Grant<\/a><\/td>\n  Must be completed each academic year.<\/td>\n<\/tr>\n\n
Meningitis\/Hepatitis B Health History Form<\/a><\/td>\n Form must be electronically completed prior to first semester of enrollment. Not a requirement for immunization; only an acknowledgement statement.<\/td>\n<\/tr>\n\n
Submit Fee Discount Forms<\/a> (if applicable)<\/td>\n Complete a fee discount form – for dependents of MTSU or State of Tennessee employees.<\/td>\n<\/tr>\n\n
Dual Enrollment Consent Form<\/td>\n Download the form here and return to the Dual Enrollment office or your high school counselor<\/a><\/td>\n<\/tr>\n\n<\/tbody><\/table><\/figure>\n\n\n\n\n
